Our office policy is quite simple:

1.If a referral is required it must be presented at the time of your visit.

2.Your co-pay must be paid at the time of your visit.

3.Patients are seen by appointment only except for emergencies.

4.Office hours are Tuesday PM, Wednesday AM, and Thursday PM. 
(We can make arrangements for other times if the above are inconvenient.)

5.A fee may be charged for an unreasonable amount of forms to be completed.
