Our office policy is quite simple:

1. If a referral is required it must be presented at the time of your visit.
    
2. Your co-pay must be paid at the time of your visit.
    
3. Patients are seen by appointment only except for emergencies.
    
4. A fee may be charged for an unreasonable amount of forms to be completed.
    
5. Bring all medical record pertaining to your problem
